1. Job Function/Purpose:

The Licensed Practical Nurse (LPN) is a member of the Home and Community Care Team of the Gwa’sala-‘Nakwaxda’xw Health department and performs assessments, plans and provides personal care, and performs nursing procedures. The LPN operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the BC College of Nursing Professionals.  The LPN is accountable to the Health Director and the H&CC RN.

2. Duties:

It is important to note that these represent the range of potential activities that can be carried out through this position.  It is necessary to prioritize which activities will actually be provided based on community needs, worker time and existing resources.

• Assesses client/family’s actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs. Where appropriate, encourages client participation in activities of daily living.
• Contributes to the development and ongoing changes to the individualized plan of care, including attending and providing input into multidisciplinary care planning meetings and family conferences.
• Performs nursing care and procedures and evaluates outcomes.
• Receives and sends referrals according to agency operating policies and procedures.
• Documents and updates all relevant information about care and procedures to achieve patient safety and comfort.
• Assists in the orientation and continuing education of new staff, students, and other members of the Home and Community Care Team.
• Participates in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings and committees.

3. Job Qualifications/Employment Requirements:

Education:

• Graduate of a recognized practical nursing program or equivalent. Current, full licensure with the BC College of Nursing Professionals.

Knowledge

• Communicate effectively, both verbally and in writing, with residents, families, co-workers, members of the multidisciplinary team and the public.
• Deal with others effectively.
• Organize work and implement a plan of care.
• Demonstrate knowledge and competencies to work at full scope of practice as currently required by the CLPNBC, including administering medications.
• Operate related equipment.
• Physically carry out the duties of the position

Experience:

• Two years of practical nursing experience in a Home and Community care program or setting.
• Must pass Vulnerable Persons Criminal Record Check

4. Working Conditions:

• Week days, 35 hours per week; some evening or week-end work may be required;
• Some travel may be required;
• The position adheres to the personnel policies and procedures, code of ethics and current job description of the Gwa`sala-`Nakwaxda`xw Nations.
